Athletic Clearance & Physicals need to be completed PRIOR to sports participation. You will receive an email once your student has been "cleared".

*NOTE: Medical Eligibility Form must be stamped by MD/DO/NP/PA or Medical Facility.
OR Medical providers electronic Physical Exam forms are accepted.
Sports Physicals are valid for one year of exam date, Kaiser electronic forms are valid for two years.
