• Download Work Permit Application (State of California Statement of intent to employ a minor and request for work permit)

• Student - Complete Minor's Information

• Parent/Legal Guardian - Print Name, Sign, & Date Application

• Employer - Complete & Signs the Business Section

E-mail the completed Work Permit Application to mchuidian@wusd.org, Melissa Chuidian, WHS Attendance & Welfare, 707-687-9499. Please allow a 24-48 hour turnaround time to get your completed work permit. You can scan a photo of your work permit application to mchuidian@wusd.org and she will send you back your Work Permit.

The official work permit will be emailed to the student's google school email address.

E-mail the completed Work Permit Application to bmorgan@wusd.org, Barbara Morgan, WHS Registrar, 707-687-9689. Turnaround time may be longer due to staff vacation/time off to get your completed work permit. You can scan a photo of your work permit application to bmorgan@wusd.org and she will send you back your Work Permit.

The official work permit will be emailed to the student's google school email address.
